Nishikori struggled to keep his Portuguese opponent in check as he lost the first set and immediately fell behind a break in the second.
However, the Japanese regained his composure and won his opening game for the seventh time at the Indian Wells Masters.
The 31-year-old is in a difficult phase of the draw and will next play against Dan Evans, who was ranked 18th.
